Registration & Appointment Information
Please note that pre-registration is required. Walk-ins will not be served. Any eligible Hoosier can self-register for a vaccine appointment. A friend or family member also can make an appointment on behalf of a qualifying individual. But the person registered will be required to provide photo identification before they will be given the COVID-19 immunization.

Registration By Phone
Eligible Hoosiers who do not have internet access or are otherwise unable to register online can call 2-1-1 for an appointment and/or secure transportation. A friend or family member also can also call 2-1-1 to make an appointment on behalf of a qualifying individual. But, the person registered will be required to provide photo identification before they will be given the vaccine.
Quick Tip
When you go to the vaccination registration site, you will be asked to enter your zip code. If you want to be vaccinated at a specific location, enter the zip code of that location. Your desired location should then appear near the top of the list of options. It will save you a lot of time of scrolling down the various options before you find the one you want.
Vaccinations for 5 thru 17 Year Olds
Only the Pfizer vaccine is approved for 5 and 17 year olds. If you are setting up an appointment for a 5 to 17 year old, please be sure to choose a vaccination site that offers the Pfizer vaccine.
